I, hereby submit voluntarily at my own discretion my Aadhaar Number/Mobile Number /Biometric details to Federal Bank and voluntarily give my consent to Federal Bank to obtain and use my Aadhaar Number, Mobile Number and/or Biometric details or One Time Pin (OTP) for Aadhaar based authentication with UDAI for the purpose of establishing/authenticating my identity for generation of Digital Life Certificate.
I understand that my Aadhar number/ Mobile Number/Biometrics and/or OTP submitted for authentication shall be used by Federal Bank only for authenticating my identity through the Aadhaar Authentication system for the purpose mentioned above or as per the requirements of law and shall not be used for any other purpose.
